Please Note:
We understand that busy schedules sometimes necessitate having a friend or relative bring your pet in, however, this puts us in a difficult situation regarding consent for services. If you need to have someone else bring your pet in, you will need to fill out a Medical Power of Attorney form provided by our office.
All patient paperwork and consent forms must be signed by an adult at least 18 years old. If the owner is under 18, a parent or legal guardian must be on records as the co-owner until the owner reaches 18 years old.
